What To Expect
During your visit, you will be greeted by one of our friendly staff team members. They will collect your completed patient forms, insurance card, insurance information and referral (if applicable), and your photo identification. You will then be called back to the exam room where we will review your medical history, be sure to provide us with a list of any previous surgeries and medical problems, current medications, and any allergies to medications. Your board-certified doctor will examine you and review your medical records, be sure to bring any x-rays, scans, and pertinent test results. The last step is to agree on a treatment plan with your doctor.
When you come for your appointment, please remember to bring the following:
• List of previous surgeries and medical problems
• List of current medication and allergies you may have to medication
• Name and address of referring and family physician
• Insurance information and referral (if applicable)
• All x-rays, scans, and pertinent test results
• Insurance Card
• Photo Identification
If you are bringing a minor for treatment you must be the legal guardian have supporting paperwork that gives you the right to make medical decisions for the child. No minor children will be seen without an adult with the right to make medical decisions for the child.
Upon your first visit to our office, it is necessary that we have all previous X-rays and medical records which pertain to your orthopaedic problem at the same time as your appointment.
Upon completion of your evaluation, a report concerning your evaluation will be sent routinely to the referring physician. If other reports are requested please let the assistant know at this time. You will be requested to proceed to the check-out counter located in the lobby.
